I think it is less an issue of ignorance than one of sensitivity. Do you wince at cartoon anvils falling on head? Perhaps this is how desensitized this person is to simulated violence.

There is perhaps another issue here with regards to judgments on what degree a person should cope with said scars as opposed to expecting everyone else to treat them as damaged goods.
